Perms, Scalp and Grey Hair
Two subjects that belong on one page: what a perm does to the skin it sits on, and what it does to hair that has stopped making pigment — because both are misread in the same direction.
The scalp half of this page is written as much for stylists as for clients. The occupational risk in perming falls on the person holding the bottle, not the person in the chair.

Two reactions that feel similar and are not the same problem
When a client reports burning, itching or soreness during or after a perm, it is almost always one of two things. They present alike and they are different problems with opposite consequences: one means a gentler chemistry next time, the other means that client can never have that chemical again, at any strength, for the rest of their life.
Irritant contact dermatitis
A barrier injury
Not immunological. A direct chemical insult to the skin barrier: an alkaline lotion at around pH 9 strips the acid mantle and the sebum layer protecting the skin, and the cells beneath swell and inflame.
It shows up during the service or within hours, as redness, stinging and mild flaking over the following days. It is dose-dependent — the stronger the lotion and the longer the contact, the worse it is — which means it is manageable. A milder chemistry, better barrier protection and less contact time genuinely fix it.
Allergic contact dermatitis
A true allergy
Immunological. The immune system has identified a specific molecule as a threat, and in perming that molecule is usually the reducing agent. It is a Type IV delayed hypersensitivity reaction, mediated by T-cells.
It takes 48 to 72 hours to appear fully: rash, blistering, severe itching, weeping. It is not dose-dependent in any useful sense. Once the memory T-cells exist, every subsequent exposure can trigger it, and the reactions tend to escalate.
The practical consequence of that delay is the part most often missed: a client who leaves the salon comfortable has not been cleared. The reaction that matters arrives after they have gone home.
What is not a salon matter
This page describes salon practice, not clinical diagnosis. A reaction that persists, spreads, blisters or weeps needs a GP or a dermatologist, not a remedy from a stylist.
Swelling of the face, lips or throat, or any difficulty breathing, is an emergency and needs emergency medical help immediately. Nothing here is a substitute for medical advice.
Glyceryl monothioglycolate, and whose problem it really is
Allergies to perm lotion are acquired. Sensitisation is a process: with repeated exposure the immune system may reclassify a molecule as a threat, and once it has, the change is permanent. That process needs repetition, which is why the population most at risk from perm chemistry is not clients. It is operators.
The most notorious sensitiser in cold waving is glyceryl monothioglycolate, the active in true acid waves, and it produces one of the more useful inversions in this trade.
Acid waves are gentler on the skin barrier and considerably more dangerous to the immune system. They run at a much lower pH and cause markedly less irritant dermatitis than an alkaline ammonia wave, so they feel kinder in the chair — while the molecule doing the work is a substantially more potent allergen than ammonium thioglycolate. The product that stings less is the product more likely to end a career.
Three things about it that are not general knowledge
- It goes through gloves. Glyceryl monothioglycolate penetrates most of the glove fabrics used in salons, including the thin disposables that a stylist changes between clients. Household-weight neoprene is the material that actually holds it out. A box of thin nitrile is protection against mess, not against this.
- It stays in the hair. It is difficult to rinse out completely, and traces persist after the service — a prolonged low-level exposure for the client, and a head of hair that goes on exposing whoever handles it at the next cut or blow-dry, for months, in salons that never applied it.
- It ends careers, not appointments. Occupational hand and forearm dermatitis is an established risk of the trade, and a sensitised stylist may lose the ability to perform the service at all. The first sensitisation paper appeared in Contact Dermatitis in November 1981; the product was voluntarily withdrawn from the German market around 1992.
What a patch test does, and the three things it does not
A patch test, or allergy alert test, is the main tool for preventing a severe allergic reaction. A small quantity of product is applied to a sensitive area of skin — the inner elbow or behind the ear — left to dry, and left unwashed and uncovered for 48 hours. Any redness, swelling, blistering or intense itching at the site means the service is refused. No exceptions, and no negotiating down to a milder product on the day.
How the test material is prepared varies, and the manufacturer’s protocol for that specific product governs it. There is no generic house method worth publishing, and an insurer will ask which protocol was followed.
What matters more is what the test does not do, and this is the most useful paragraph on the page.

A clear test is not a guarantee
It substantially reduces risk. It does not eliminate it. A negative result means no reaction was detectable at that site, at that concentration, on that day.

A clear test does not travel
It does not cover future services indefinitely and it does not transfer to a different product. A test on one manufacturer’s lotion says nothing about another’s.
Three
A long clear history proves nothing
A client who has had ten uneventful perms is not thereby safe. Sensitisation develops through exposure. An uneventful history is how a sensitisation is built, not evidence against one.
That third point is the one clients and stylists get wrong most often. “I have been having these for twenty years” is a description of accumulated exposure, not a safety record.
And being honest about salon practice: the trade has been rigorous about patch testing for colour, because of PPD, and considerably laxer about perm lotions. Where there is any history of skin sensitivity, or a glyceryl monothioglycolate wave is going on somebody new, testing is not optional.
Do not scrub, scratch or exfoliate the scalp beforehand
Because irritant dermatitis is a barrier injury, the state of the barrier when the lotion goes on decides how the day goes. For a couple of days before a perm, leave the scalp alone: no scalp scrubs, no exfoliating treatments, no vigorous scratching, and no picking.
An alkaline lotion on an intact scalp is an irritation. The same lotion on a micro-abraded scalp is a burn.
Natural sebum is a barrier against the alkalinity of the lotion and it is worth having on the day. The pre-service wash is a clarifying wash of the hair, which does need clearing of silicone and mineral build-up so the lotion diffuses evenly. It is not a scrubbing scalp massage. Clean the fibre, leave the skin.
Tension does the same damage mechanically. Even tension is required when wrapping, but excessive tension pulls on the follicle and irritates the scalp before any chemistry arrives — and a stylist reaching for more of it is usually compensating for a rod that is the wrong size, which is a sizing problem being solved on the client’s skin.
Afterwards
A scalp that has taken mild irritant dermatitis stays tender for several days. No scrubbing, no scratching, and no dragging a heavy multi-row styling brush across it — that micro-tears compromised skin and fractures newly oxidised bonds at the root at the same time. Wide-tooth combs until the acid mantle is back, roughly the three days the oxidation itself takes.

The scalp
Active skin conditions
Active psoriasis, severe eczema, open sores, an already inflamed scalp. The service is refused. That the hair itself would take the perm is beside the point.

The chemical limits
On bleached hair the answer is usually no — highlights and foils included, and grown-out bleach still on the lengths included. Bleaching leaves cysteic acid, and a neutraliser needs two sulphur atoms it can no longer find, so that is the default and the default is no; the exception is a judgement made on the hair in front of me rather than a rule read off a page. Hydroxide-relaxed hair is never permed. Single-process tint can be, with a strand test taken first and the lotion strength downgraded for the raised porosity. Hair that has stopped making pigment is resistant, not fragile
The commonest error made on grey and white hair is treating it as delicate. It is generally the opposite. What people read as fragility is coarseness and wiriness, and the actual behaviour under chemistry is stubborn resistance.
What is reliably observed at the chair: white hair tends to be low in internal moisture and lipids, which is what gives it that dry, wiry handle, while its cuticle scales are tightly overlapping and well fortified with the hydrophobic lipid layer — principally 18-MEA — that low porosity measures. Elasticity is often perfectly strong. It behaves as a physical barrier, so treat it as though it were fine hair and the lotion sits on the surface, never reaches the cortex, and the perm drops out within days.
Which is why the strongest lotions are indicated on the hair most people assume needs the mildest. For coarse, wiry, resistant white hair, an alkaline wave at about 9 to 9.5 is usually the only thing that will do the job, because the alkalinity is what opens a tight cuticle at all. That is a diagnosis of low porosity, not an aggressive choice.
The trap inside that, though
Grey hair is rarely uniform. A client may have pure white at the temples — exceptionally coarse and resistant — sitting next to finer, more porous silver or still-pigmented hair at the nape. A single blanket strength across mixed grey fails in both directions at once: the temples under-process while the nape over-processes, on the same head, in the same twenty minutes. Staggered application timing or porosity equalisers are not optional here, and this is the case where a stylist earns the fee.
The mechanics change too. Coarse white hair needs firm, even tension to hold the bonds in their new alignment; where it has gone dry and brittle with age, over-tensioning snaps it at the root. It is also why a large-rod wave disappoints so often here — hair lacking internal pliability fights a shallow bend and lets it go almost at once. Where a client with resistant white hair wants softness rather than curl, the honest answer is a smaller rod worked loosely, not a large rod worked tight.
One thing does not soften for white hair: bleached white hair is still bleached hair, and the answer there is still usually no. Bleaching to brighten grey is common and it leaves exactly the same cysteic acid behind as bleaching anything else. Resistant and bleached is still bleached.
The yellow cast, and where it actually comes from
The aesthetic risk unique to unpigmented hair is discolouration. With no pigment to mask anything, every alteration to the fibre shows, and a perm can turn a clean white head yellow in an afternoon. Two causes, and neither is the lotion itself.
Cause one
Over-oxidation
An oxidation step that is rushed, uneven, or run with degraded hydrogen peroxide leaves residues in the fibre that go on reacting with light and air, and the result is a yellow cast that arrives over days rather than in the chair. Excess thermal input does the same thing by a different route: white hair proteins are notably heat-sensitive, and keratin given more heat than it needs scorches and yellows.
Cause two
Trapped metals
White hair readily absorbs environmental contaminants — tobacco smoke, pollution, and hard-water minerals, particularly iron and copper. When an alkaline lotion or a peroxide neutraliser meets those trapped metals, the oxidation reaction turns bright white hair yellow immediately. In London, on some of the hardest water in the country, this is not a rare case. It is the default risk.
The answer to the second is a thorough clarifying and chelating wash before the service, and there is no substitute for it on a white head — what such a treatment can and cannot reach is set out on curl memory and water. Afterwards, violet-toned conditioning and steady hydration keep permed white hair clear rather than dull, and they belong in the plan at the start rather than as an apology at the end.
Questions
How do I tell the difference between irritation and an allergy?
Mostly by timing. Irritation arrives during the service or within hours and settles over days. An allergic reaction arrives 48 to 72 hours later and gets worse rather than better. If a reaction appears two or three days later, take it to a doctor and tell the salon, because it changes what you can have done for the rest of your life.
I have had perms for years with no trouble. Do I still need a patch test?
Yes, and your history is the reason rather than the exception. Sensitisation is built by repeated exposure, so twenty uneventful years is a description of the exposure, not a guarantee about the next one. It is also why a test on one product does not cover a different one.
Is an acid perm the safer option if my scalp is sensitive?
For irritation, often yes — it runs at a much lower pH and is far less harsh on the barrier. For allergy it can be the riskier one, because glyceryl monothioglycolate is the more potent sensitiser. Two different questions, two different answers.
Is grey hair too weak to perm?
Usually the reverse. Unpigmented hair tends to have a tight cuticle, low porosity and perfectly good elasticity, so it resists the lotion rather than surrendering to it — which is why it often takes a stronger wave than the same client would have needed twenty years ago. The exception is grey hair that has also been lightened, and that is usually a refusal.
Why did my white hair go yellow after a perm?
Almost always an over-oxidised or over-heated fibre, or metals already in the hair reacting with the chemistry. Hard-water iron and copper are the usual culprits, and a chelating wash before the service is what prevents it. Once the yellow is in, toning manages it rather than undoing it.
